Complete this form to submit your charity, then email your video to [email protected]!
STEP 1
Think of a charity or non-profit organization you would like to nominate and why you believe this organization should receive the $1,000 donation. The charity must have a 501(c3).
STEP 2
Get a video of the organization in action. It could be a video of the staff dancing in front of their logo or some footage showing them doing good in the community.
STEP 3
Send us your video by completing the form above. You can upload to Dropbox or YouTube... just make sure it's the raw/original footage and it's easy for us download.
STEP 4
Let's celebrate! If your charity is chosen, we will give you a shout out by featuring your video on our social media, and we'll notify you so you can share with the organization!
